Overdraft Services Opt-In/Opt-Out Authorization

Fremont Bank will not authorize and pay overdrafts for ATM and everyday debit card transactions on your checking account unless you provide us authorization.

If you would like to have your transactions approved, at our discretion, complete and submit your Opt-In consent below. You may also use this form at any time to revoke your authorization (Opt-Out).


By submitting the form below you authorize (“Opt-In”) Fremont Bank to pay overdrafts on your ATM transactions and everyday debit card transactions for each checking account listed below.

Important Overdraft Privilege Program Details
Overdrafts may be created by an ATM transaction, debit card transaction or other electronic means. An overdraft fee per item may be charged, up to a maximum limit per day. Whether your overdraft is paid is at Fremont Bank’s discretion, and we reserve the right not to pay. For example we typically do not pay overdrafts if your account is not in good standing, or you are not making regular deposits, or you have had excessive overdrafts. Once an overdraft has occurred, you are expected to bring your account to a positive balance promptly. There is a daily overdraft charge if account remains overdrawn for 10 or more consecutive business days until the account is returned to a positive balance. Fees charged will reduce overdraft protection limit. Transactions are not posted to the account in the order in which they occur and can affect the total amount of overdraft fees charged.
